RELEASE & WAIVER OF LIABILITY & CONSENT

By signing this form, I acknowledge that I am aware of the risks associated with participation in gymnastics. I warrant that I, or the participant named on this registration form, is physically and mentally fit to participate in gymnastics activities. I understand that in athletic activities there is the possibility of injuries. I, the undersigned, hold harmless TAG Performance Studio (TAG) and its instructors for any and all injuries arising out of participation in any and all classes, practices, free play, demonstration or competitions away or at the TAG facility and during travel to and from these events. I release TAG and its directors, officers, employees, volunteers and agents from and against any and all causes of actions, claims, demands & liability that I, or the participant named on this registration form, our respective assignees, heirs or executors may have now or in the future against TAG related to my participation in gymnastics activities at TAG.

MEDICAL CONSENT

I, the undersigned parent or guardian of the above-named student do herby grant authority to the staff of the TAG to render judgment concerning medical assistance in the event of an accident or illness during my absence. I give consent for the TAG staff to obtain emergency medical assistance and treatment as necessary.

IMAGE RELEASE

Participants at any event in which TAG is involved may have their image, likeness, name (excluding personal address, phone and/or email address), club, province, city/town, as well as rank within Canada and previous performing competitive, judging choreographing or coaching history used in publication and on the internet by TAG as well as its agents from time to time. When signing this form, gymnasts, volunteers, coaches, judges and in the case of minors, their parents/guardian, agree that they have the authority to provide this authorization/approval. A scanned and emailed copy, or a photocopy of this form shall be deemed to constitute an original signed document. I allow the use of personal information as outlined above and image in TAG media including newsletters, website, poster, brochure, video, sponsorship package or general advertising.

I acknowledge that I have carefully read this Release and Waiver of Liability and Consent and understand its contents.
